Application of director, officer or employee of a member b a n k for permission of the F e d e r a l R e s e r v e B o a r d to s e r v e a t the same time as director, officer or employee of not more than two other b a n k i n g institutions u n d e r t h e K e r n A m e n d m e n t to t h e C l a y t o n A c t ; F o r m 9 4 ( a ) S t a t e m e n t to b e s u b m i t t e d i n c o n n e c t i o n with such applications by each hanking institution involved; F o r m 9 4 ( b ) S t a t e m e n t to b e s u b m i t t e d b y F e d e r a l E e s e r v e Agent w i t h reference to such applications.
